How to fill it in
- Copy each vaccine from the vet's card: the date, the vaccine's name as printed, and the next due date the vet wrote.
- At each visit, ask the vet to stick the label from the vaccine vial on the card. Vet guidelines recommend peel-off vaccine labels for exactly this kind of record 6. Copy the batch number from that label.
- Write the vet's name and council registration number. MCD's sample certificate asks for this number 1.

What the vet's certificate should show
- Printed on the letterhead of a registered vet, with the vet's signature and stamp 1.
- A photo of the dog with its owner, attested by the vet 1.
- The owner's name, address and mobile number, and the dog's name, gender, breed, colour and any identification mark 1.
- The date of the rabies vaccination, the next due date, and the vet's council registration number 1.
- In Mumbai, BMC wants rabies and leptospirosis certificates, signed and stamped 3.
- WSAVA, the world association of small-animal vets, suggests a certificate also say how long each vaccine is expected to protect. If yours doesn't, ask the vet to write the next due date 6.
